ecent advances in mechanical ventilation

چکیده

Important advances have been made over the past decade towards understanding the optimal approach to ventilating patients with acute respiratory failure. Evidence now supports the use of noninvasive positive pressure ventilation in selected patients with hypercapnic respiratory failure and chronic obstructive pulmonary disease, cardiogenic pulmonary edema, and for facilitating the discontinuation of ventilatory support in patients with chronic pulmonary disease. The concept of a lung protective ventilatory strategy has revolutionized the management of the acute respiratory distress syndrome. The process of liberation from mechanical ventilation is becoming more standardized, with evidence supporting daily trials of spontaneous breathing in all suitable mechanically ventilated patients. This article critically reviews the most important recent advances in mechanical ventilation and suggests future directions for further research in the field. © 2005 Elsevier Inc. New modalities of mechanical ventilation: high-frequency oscillatory ventilation and airway pressure release ventilation.

Management of acute lung injury (ALI) and the acute respiratory distress syndrome (ARDS) is largely supportive, with the use of mechanical ventilation being a central feature. Recent advances in the understanding of ALI/ARDS and mechanical ventilation have revealed that lung-protective ventilation strategies may attenuate ventilator-associated lung injury and improve patient morbidity/mortality...

Recent advances in paediatric ventilation.

BACKGROUND To review the recent advances in ventilatory therapy for acute respiratory failure in children. DATA SOURCES Recent published peer-review articles on mechanical ventilation for acute respiratory failure in children. SUMMARY OF REVIEW Advances in conventional treatment for acute respiratory failure (e.g. mechanical ventilation) have not increased survival in children. However, rec...

Comparison of the effect of pressure support ventilation and volume assured pressure support ventilation on weaning patients off mechanical ventilation after cardiac surgery

Background: Weaning from mechanical ventilation in the intensive care unit is an important step in the treatment process. More patients in less than 6 hours after cardiac surgery can be separated from mechanical ventilation and extubated. However, 20-40% of patients after cardiac surgery, due to dysfunctional ventilator weaning response (DVWR), still remain under mechanical ventilation. Therefo...
